- Your name, mailing address, telephone number, and email.
- A statement that you have a good-faith belief that the disputed use is not authorized by the copyright owner, its agent, or law.
- A statement, made under penalty of perjury, that the information in the notice is accurate and that you are the copyright owner or authorized to act on behalf of the owner.

Counter-Notice
If material you posted was removed and you believe the removal was a mistake or misidentification, you may file a counter-notice including (a) your signature, (b) the material removed and where it appeared, (c) a statement under penalty of perjury that the removal was a mistake, and (d) your contact information and consent to jurisdiction in California.
